In Memory of Mario Medero M.D. On July 25th, 2008, after 9 years of coping with the ALS disease , at the age of 56, he passed away.

Our physician's at each facility are constantly kept up to date with new developments in 'Workers Compensation' legislative guidelines and receive extensive training in all aspects of occupational medicine procedures including safety and preventative care.

Through interactive communication between our physicians, employers and carriers, Medero Medical maintains consistent access to information concerning the injured worker's return to work status. Following all initial injury evaluations and treatment our physician will communicate with the employer to discuss diagnosis, treatment plan, and work duty status of the injured worker. This procedure insures the employer is fully informed throughout the continuum of care and our physicians are available for follow up to answer any return to work concerns the employer may have.

In Memory of

Mario Medero, M.D.
is a graduate of the University of South Florida, Tampa, where he completed his medical degree. He went on to do his internship and residency in General Surgery at the University of Florida, Gainesville. Dr. Medero began his career in Occupational Medicine in Ocala, Florida in 1984. He is the founder and President of Medero Medical. Dr. Medero is fluent in the Spanish language. He is a Certified Medical Review Officer and a Certified Independent Medical Review Examiner.

The University of South Florida's Occupational Medicine Residency Program has selected Medero Medical as a training facility for their residents. Dr. Medero acts as the teaching physician, utilizing his many years of experience in Occupational Medicine, trauma, wound care and surgery with these physicians in all six facilities. He enjoys spending time with his family.
Ilk A. Fahey, M.D., Medical Director, Medero Medical of Orange, LLC. is Board Certified, Urgent Care Medicine from the American Board of Urgent Care Medicine. She earned her bachelors degree from the University of South Florida with honors and as a member of the Golden Key National Honor Society. Dr. Fahey went on to complete her medical degree at Tufts University School of Medicine in Boston, Massachusetts. She then completed her internship with an emphasis on family practice and emergency medicine at the Medical Center of Central Georgia. Dr. Fahey then completed her surgical residency in Gynecology at Mercer University in Macon, Georgia. At Mercer University she was the recipient of the 2000 Gold Foundation Humanism and Excellence in Teaching Award.

Dr. Fahey is a certified Medical Review Officer. She is fluent in the Spanish language. Away from the office, Dr. Fahey enjoys spending time with her husband, her three children and her two dogs. She is also an avid saltwater aquarist and enjoys gardening.
Edward L. Demmi, M.D. is a graduate from the University of South Florida. He completed his training in Internal Medicine at the University of Alabama. He has been practicing occupational medicine for more than 20 years and is a member of the American College of Occupational & Environmental Medicine. He is also a certified Aviation Medical Examiner, a certified Medical Review Officer, Certified Substance Abuse Professional and a member in the Marion County Medical Society. Dr. Demmi has been appointed by the University of South Florida, Department of Medicine, Branch of Preventive Medicine as Assistant Associate Professor of Medicine. He currently holds joint faculty appointments at the University of South Florida within the College of Medicine and College of Public Health. He is actively involved educating and training other doctors in the clinical and administrative practice of occupational medicine. He is fluent in both Spanish and Italian. Dr. Demmi is currently the Corporate Medical Director of Medero Medical.

Away from practicing medicine Dr. Demmi enjoys the outdoors and is an avid flats fisherman. He has been published in the Florida Sportsman Magazine for landing a 20-pound snook. He can often be found fishing the Boca Grande waters off the coast of Sarasota, Florida.
Nagy Shanawany, M.D. received his undergraduate, graduate and medical degrees from Cairo University, finishing in 1967. His training was in general surgery and he served as a general surgeon at Tripoli Central Hospital for many years before arriving in Florida. Dr. Shanawany has worked in the fields of occupational medicine and urgent care in Tallahassee, Ocala and Leesburg, Florida since 1986. He is certified by the American Board of Quality Assurance and Review Physicians, he is a Certified Medical Review Officer and he is an active member of the Lake County Medical Society. He is also a current member of the Marion County Medical Strike Team.

Dr. Shanawany is married with two children. He and his wife enjoy traveling and sightseeing throughout the southeast. At any weekend, you will find him and his wife at the Disney attractions, especially Epcot. Today, Dr. Shanawani is the Medical Director of Medero Medical Of Lake, LLC.
Maureen Levy, M.D., MBA, MHA is a graduate from American University, Washington, D.C. She continued her training at the University of Medicine and Dentistry of New Jersey, Rutgers Medical School, now known as Robert Wood Johnson Medical School. She completed her residency in Internal Medicine at Brookdale University Medical Center and trained in Anesthesia and Pain Management at the Cleveland Clinic Foundation. Dr. Levy practiced internal Medicine and Emergency Medicine in New York and Pain Management in Florida for a combined period of 12 years. She received her MBA at the University of California, Irvine and her MHA at University of Florida, Gainesville. She is a Certified Medical Review Officer. Dr. Levy is an avid reader and enjoys traveling.
Owen Smith, M.D. is the Medical Director of Medero Medical Tampa East. He earned a Bachelor of Science from Cornell University, Ithaca, New York and received his medical degree from Universidad del Noreste in Mexico. He continued his residency training in Internal Medicine at Brookdale University Medical Center, Brooklyn, New York and became a Board Certified Internist. Dr. Smith practiced internal medicine and emergency medicine in New York before moving to Florida. He is a certified Medical Review Officer.

When not practicing medicine, Dr. Smith spends time with his wife, two children and three dogs. He enjoys fishing, gardening and watching both the Tampa Bay Lightening hockey team and his son play hockey.
Samer Choksi, M.D., M.P.H. is the Medical Director at Medero Medical of Marion. He is a board certified physician through the American Board of Preventive Medicine in Occupational & Environmental Medicine. In addition, he is a graduate of the University of South Florida Advanced Sub-Specialty Residency Program in Occupational & Environmental Medicine. Moreover, he has completed his undergraduate studies at Jacksonville University and graduated with honors with a major in Biology and double minors in Chemistry and History. While completing his undergraduate degree, Dr. Choksi’s passion for public health led him to further his education to include a Master’s of Public Health in Infectious Disease Epidemiology at Tulane University in New Orleans, LA. As he pursued this degree, Dr. Choksi was involved in HIV research which ultimately prompted him to pursue a career in medicine which he completed at The Medical College of Wisconsin in 2002.

Dr. Choksi’s commitment to education and research is evident as he currently holds joint faculty appointments at the University of South Florida within the College of Medicine and College of Public Health. He is actively involved educating and training other doctors in the clinical and administrative practice of occupational medicine.

Outside of Medero Medical, Dr. Choksi keeps busy with an active fitness program, traveling internationally, and enjoys the heritage and historic nature of Ybor City, Tampa, Florida.
Carol R. Grant, M.D. is the Medical Director of Medero Medical Orange South. She is a graduate of the Medical School of University of the West Indies in Jamaica. She completed residency training in both Internal Medicine and Pediatrics at the medical School of the University of Medicine and Dentistry in New Jersey in 1995. Since then she has practiced in New Jersey and Florida in both the Emergency Room and Urgent Care settings. Dr. Grant is a certified Medical Review Officer (MRO).

She is married and is the mother of two boys. She enjoys spending time off traveling internationally with her family.
